New York State Public Health Law requires students to get certain vaccines to attend childcare or school. In June, all students should have received all required immunizations. Please take a look at this updated immunization requirements letter (includes translations under “Immunization Chart for Families”). We encourage families to take time this summer to review their children’s immunization history with their healthcare providers, so that they may be up-to-date for the start of the school year.

In accordance with Chancellor's Regulation A-701, all students entering DOE public schools for the first time, from kindergarten through grade 12, must submit a New Admission Examination form (CH-205)–completed by a health care provider–to their school nurse as soon as possible after registration.

Any students needing to take medications or receive any medical treatments must submit a Medication Administration Form (MAF) and/or Medically Prescribed Treatment Form (scroll down on the page) every year. For more information on health services in school, visit the Health Services page.
